UC Riverside students, employees, and visitors are required to fill out the Daily Wellness Check on all days when they will be in campus owned or operated facilities. It also will be required for many events and gatherings on campus.
Accessing the Daily Wellness Check
Anyone filling out the Daily Wellness Check will provide information about recent contact with someone who has tested positive for COVID-19, travel outside California, COVID-19 symptoms, and vaccination status. Once the survey is completed a notification will be provided either granting a level of clearance to be on campus or a request to stay home and take additional steps to gain clearance to be campus.

UCR will keep information about an individual's symptoms confidential, accessible only to those with a need to know in order to address the situation. The information will not be maintained in personnel files.
What Happens Next
The UCR Case Investigation Team will follow up on any reports of exposure or a positive test from students or employees on the Daily Wellness Check. More information about the case investigation process can be found here.

You do not need to fill out the Wellness Check when working remotely, unless you need to report a positive case, exposure, or COVID-19 like symptoms.
